1. The first 480 hours of employment will be an introductory period. At the completion of 480 hours, performance will be reviewed and if proven to be satisfactory, the employee will be placed on continued active status. (Type initials here) *
2. Employees that represent SARA Companion HomeCare Services (hereinafter referred to as " SCHCS ") are expected to show up dressed appropriately in clean, solid, or multicolored non-offensive, non-discriminatory scrubs, printed smocks, sneakers, or nursing shoes unless advised otherwise. 3. If the client has a landline phone it can only be used to clock in/out and only if you are unable to use the Mobile App. At no time should personal calls be made on the client’s home or cell phone. Employees are NOT allowed to make personal calls on duty. Emergency calls should be brief otherwise it would be a VIOLATION OF THESE CONDITIONS. (initial here) *
4. NEVER under any circumstances should you discuss any personal issues with the client and or the client’s family such as your pay rate, home situations or any other personal matters. (initial here) *
5. Employees may not accept money, clothes, gifts, or personal property from the client and or the client’s family. (initial here) *
6. You are required to give SCHCS NOT the client a MINIMUM of two (2) hours’ notice if you are going to be absent and immediate notice if you are going to be late, except where a verifiable emergency exists. Please note that EXCESSIVE ABSENTEEISM AND/OR LATENESS ARE UNACCEPTABLE AND GROUNDS FOR TERMINATION. (initial here) *
7. Please call our after-hour number at 516-837-3388 Option #2 between the hours of 5:00 pm and 9:00 am. It is also available to you when the office is closed. (initial here) *
8. Never leave a case if you experience an issue. You must notify the office immediately so we can assist with the issue and or reassign you to another case. (initial here) *
9. Always follow the care plan. If the task is not on the care plan, contact the agency before performing it. (initial here) *
10. If you solicit and/or work for any SCHCS clients privately, it will lead to termination and you will be subject to liquidated damages as stated in the liquidated damages agreement, fines and or reimbursements. (initial here) *
11. Below is a list of unacceptable actions that will lead to disciplinary action and or termination (initial here) *
• No Call/No Show (initial here) *
• Leaving a case/ abandoning a patient/negligence and any form of abuse such as physical, mental, verbal, emotional and spiritual. (initial here) *
• Falsifying clock in and out times. (initial here) *
• Failing to clock in and out (unless extenuating circumstances, staff at SCHCS will not manually clock in/out for you). (initial here) *
• Transporting patients without authorization from office personnel (initial here) *
• Insubordination/Offensive language that insults, intimidates, or is abusive to the client, their family member and or office personnel. (initial here) *
• Failure to comply with written/verbal instructions or assigned duties/plan of care. (initial here) *
• Unexcused excessive tardiness & absenteeism. (initial here) *
• Failure to report a change in your patient’s health status. (initial here) *
• Being under the influence of alcohol or illegal drugs while at the agency or the client's home including smoking cigarettes, cigars, marijuana, hookah, or vaping. (initial here) *
• Disregarding HIPAA and or other safety regulations. (initial here) *
• Theft, destruction or damage or misuse of any property belonging to the client and or family. (initial here) *
• Performance that doesn’t meet SARA’s expectations or requirements. (initial here) *
• Participation in activities on and off the premises that may be inconsistent with the Code of Conduct expected by all employees. (initial here) *
• Sexual harassment or harassment of any kind. (initial here) *
• Allowing your friends or family members to enter a patient’s home or property. (initial here) *
• Immoral conduct or indecency at the agency and the client’s home. (initial here) *
• If you are removed from a case or terminated for ANY reason listed above, you agree that you will have no further contact with the client and or their family members. (initial here) *
